Menthol is a familiar ingredient in many over-the-counter creams, gels, balms, and patches for temporary pain relief. When applied to the skin, it produces a characteristic cooling sensation that can make an aching area feel more comfortable. For adults living with chronic back, muscle, or joint pain, understanding how does menthol work in creams for chronic pain can help clarify what these creams can and cannot do as part of a broader pain-management plan.
Menthol primarily affects sensory receptors in the skin rather than directly treating the underlying cause of chronic pain. One of its main targets is a receptor called transient receptor potential melastatin 8, or TRPM8. These receptors are activated by cool temperatures as well as by compounds such as menthol. When menthol stimulates TRPM8, nerve cells send signals that the brain interprets as cooling, even though the temperature of the skin may not have changed substantially. Research suggests that this activity can influence the way pain signals are perceived and may contribute to menthol’s analgesic, or pain-relieving, effects.
Menthol is often described as a counterirritant. In simple terms, a counterirritant creates a noticeable sensation at the skin’s surface that can temporarily alter or distract from deeper discomfort. The cooling sensation may initially stimulate sensory nerves and then reduce their responsiveness. Menthol may also affect additional nerve-signaling pathways involved in pain. Its effects can depend on concentration, however, and higher concentrations do not necessarily provide better relief. In some circumstances, stronger exposure can increase sensitivity to cold or cause irritation.
This mechanism helps explain why menthol creams may feel helpful without actually correcting an injured joint, compressed nerve, herniated disc, arthritis, or another condition responsible for chronic pain. Menthol-containing topical analgesics are generally intended to provide temporary relief of minor muscle and joint aches rather than cure the source of chronic symptoms. Product formulations also differ. Some contain menthol alone, while others combine it with ingredients such as methyl salicylate, camphor, or other topical analgesics, so directions and precautions can vary from one product to another.
Topical products can be appealing because they are applied directly to the area that hurts, but they still need to be used carefully. Follow the product label and avoid applying menthol creams to wounds or broken, irritated skin. Keep them away from the eyes and other sensitive areas, and do not use heating pads over treated skin unless a healthcare professional or the product instructions specifically indicate that it is safe. The FDA has warned that rare cases of serious skin burns have occurred with some over-the-counter topical pain relievers containing menthol, methyl salicylate, or capsaicin. If severe burning, swelling, pain, or blistering develops after application, stop using the product and seek medical attention.
For chronic pain, menthol is best viewed as one possible symptom-management tool rather than a treatment strategy. Persistent back, neck, or joint pain can have many causes, and treatments that help one condition may not be appropriate for another. However, reducing pain to a more manageable level can be beneficial in increasing patients’ ability to function, with the caveat that it is still important to investigate the actual cause of chronic pain and treating it as much as possible to reduce symptoms and improve quality of life in the long term.
