Myofascial pain syndrome is a persistent musculoskeletal pain condition involving muscle and the fascia, the connective tissue that surrounds and supports them. It is commonly associated with sensitive areas known as trigger points, which may feel like tight knots or firm bands within a muscle. Pressing on a trigger point can cause pain directly at that location or produce referred pain in another part of the body. For example, a trigger point in the neck or upper back may contribute to pain in the head, shoulder, or arm.

The condition may develop when a muscle is repeatedly overloaded, held in an awkward position, or unable to recover adequately from physical stress. Potential contributors include repetitive movements, occupational strain, poor posture, muscle weakness, previous injuries, prolonged inactivity, and stress-related muscle tension. In some people, symptoms begin after an identifiable injury, while in others there is no single clear cause. Sleep disruption and ongoing emotional stress may also worsen muscle tension and increase sensitivity to pain.

Symptoms vary but often include deep, aching pain, muscle stiffness, tenderness, reduced flexibility, and discomfort that persists despite rest. The pain may become more noticeable during activity, after prolonged sitting, or when pressure is applied to the affected muscle. Some patients also experience headaches, disturbed sleep, fatigue, or limitations in everyday tasks. Unlike ordinary muscle soreness, which generally improves as tissue recovers, myofascial pain can continue and may interfere with work, exercise, and quality of life.

Diagnosis of myofascial pain syndrome is usually based on a detailed medical history and physical examination. A clinician may assess posture, strength, movement patterns, joint function, and areas of muscle tenderness. The examination may identify a taut band of muscle containing a sensitive point that reproduces the patient’s familiar pain. There is no single blood test or imaging study that confirms myofascial pain syndrome. However, testing is needed to exclude conditions such as nerve compression, joint disease, inflammatory disorders, or structural spine problems that can produce similar symptoms.

Treatment usually requires more than simply resting the painful muscle. Physical therapy and a carefully designed exercise program are central components of care. Treatment may include stretching, progressive strengthening, posture and movement retraining, manual therapy, and guidance on modifying repetitive activities. Heat, relaxation techniques, and improvements in sleep or workstation ergonomics may also help reduce factors that continually aggravate the muscles. Exercise is particularly important because it can restore normal movement and improve the muscle’s ability to tolerate activity.

Medications may be used to control symptoms, although they do not necessarily address the underlying mechanical contributors. Depending on the patient, a clinician may recommend nonprescription pain relievers, topical medications, or selected prescription treatments. For persistent trigger points, dry needling or trigger point injections may be considered as part of a broader rehabilitation plan. These procedures target the painful muscle area and may provide enough relief for the patient to participate more effectively in therapy and exercise.

Myofascial pain syndrome can often be managed successfully, but improvement may be gradual. Identifying physical stresses, restoring movement, and maintaining an appropriate exercise program are important for reducing recurrence. A physical medicine and rehabilitation or pain management evaluation can help determine whether symptoms are primarily muscular or related to another spine, joint, or nerve condition requiring different treatment.
