Injections at tendon insertion sites, tendon sheath, and bursa are used to diagnose and manage certain types of musculoskeletal pain. They are often considered when pain is linked to inflammation or irritation around tendons, the points where tendons attach to bone, or fluid-filled structures called bursae. For people living with chronic pain, these injections may form part of a broader treatment plan that can also include physical therapy, activity modification, medications, and rehabilitation.
A tendon connects muscle to bone and helps transfer the force needed for movement. The area where a tendon attaches to bone is known as the tendon insertion, or enthesis. Repetitive strain, overuse, injury, or certain inflammatory conditions can cause pain at this attachment point. Tendon insertion injections are intended to target the painful region directly. Depending on the condition and the clinician’s assessment, an injection may contain a local anesthetic, a corticosteroid, or another therapeutic substance. Because some medications can weaken tendon tissue when placed directly into a tendon, careful technique and precise placement are important.
A tendon sheath is a thin layer of tissue surrounding certain tendons, particularly those that move through narrow spaces in the hands, wrists, ankles, and feet. The sheath allows the tendon to glide smoothly during movement. When the sheath becomes inflamed, a condition known as tenosynovitis may develop. This can cause pain, swelling, stiffness, or a catching sensation. Conditions such as trigger finger and some forms of wrist pain may involve tendon sheath inflammation. An injection into the sheath can reduce inflammation and may improve movement and comfort.
Bursae are small, fluid-filled sacs located between structures such as tendons, muscles, bones, and skin. They reduce friction and cushion tissues during movement. When a bursa becomes irritated or inflamed, bursitis can result. Common joints for bursitis are the shoulder, hip, elbow, and knee. Bursa injections are designed to deliver medication directly into the affected area, with the goal of reducing inflammation and relieving pain.
These procedures are usually performed in an outpatient setting. The skin is cleaned, and the clinician identifies the correct injection site. In some cases, ultrasound guidance is used to visualize the tendon, sheath, bursa, and surrounding structures in real time. Image guidance can help improve accuracy, especially when the target is small or located near nerves, blood vessels, or other sensitive tissues.
Pain relief after an injection varies. Local anesthetic may provide temporary improvement within minutes or hours, while the effects of a corticosteroid may take several days to become noticeable. Some people experience significant relief, while others have only limited or short-term benefit. An injection does not necessarily correct the underlying mechanical cause of a tendon or bursal problem, so rehabilitation and changes in activity may remain important.
Although these injections are generally considered minimally invasive, they are not risk-free. Possible complications include temporary soreness, bleeding, infection, skin or tissue changes, allergic reactions, and, in certain circumstances, tendon damage or rupture. People taking blood-thinning medications, those with diabetes, or those with active infections may require additional precautions.
